Jacob is a family physician who has spent more than 30 years at the intersection of health care and technology. He served as Deputy National Coordinator and Acting National Coordinator for Health IT at The US Department of Health and Human Services / ONC, and as Chief Medical Informatics Officer at Allscripts. As CEO of the Alliance for Better Health, he built a value-based care model serving 150,000 Medicaid members, and as CEO of Harbor Health ACO he launched a Medicare ACO REACH program.
